That is a lot of mixed signals, not sure I’d know exactly what to make of that either. To be brutally honest though it sounds like it could be a case of her missing you while you were gone and wanting to reconnect but now that you’re actually back and in a position where you could do that she’s reconsidered, either because she’s also unsure of what she wants or she may have just been reminded of what things ended in the first place. It’s impossible to know without having a real conversation with her. I think your therapist is right and you should reach out, and ask her to meet so the two of you can talk. Before you do see her though make sure you’re absolutely sure about what you want, and don’t allow yourself to settle. If she doesn’t want to give you the kind of commitment you’re looking for you need to be able to walk away. Also something to consider is that you’d agree you’re afraid to reach out because you don’t want to get hurt, but aren’t you hurting now? If the two of you never spoke again it seems like that would hurt quite a bit too. If you’re going to hurt, you may as well do it knowing you put yourself out there and even if you didn’t get what you were hoping for you have closure now and know where you stand, which is always better than being left guessing. Hope it works out for you with or without her.
