Just another possible perspective:
If she doesn’t want to address the issues you can’t make her. You’re getting a lot of flack in here but it sounds to me like she’s probably just ignoring issues instead of tackling them and that’s very likely the way she handles all her problems. If that’s the case then it’s better that you steel your heart and break it off as this approach to problem solving is not conducive to a long term, healthy relationship.
Unfortunately I see this approach more and more often these days as people become less willing to do hard things, control themselves have discipline and sacrifice.
It’s very possible that she does have some medical or emotional issue, and that should be looked into before you up and leave. But Occom’s Razor, the only real issue is the biggest one we face as a culture today: laziness and entitlement.
Sounds like Bipolar. I would suggest she see a therapist or a doctor to look at being placed on meds to help with that. If she can’t or refuses then yea it might be good to call it off.

This. I sincerely doubt that asking/telling her to change her legal name is enforceable even if it was agreed to.
Changing your name is a hassle. That’s why I didn’t change mine when I got married. I was already established professionally and had no intention of having kids so I just kept my name.
My mom didn’t change her name after her divorce. My ex husband’s ex wife didn’t change hers. I don’t really see the problem.

Be an adult pay half the rent. Make him pay half the groceries and cleaning supplies. Talk to each other to find a fair division of chores.
There was another post a while ago that said almost the exact same thing, in the update it turned out he was telling her that so she would have low self esteem and feel like no one else would ever want her.
The word reaching springs to mind. People are paid in generic porn videos, they're not doing it for free. If they're not being paid well enough, then, again, that isn't OPs fault.
