Sounds like she has some serious negative connotations/shameful feelings towards sex likely stemming from growing up in a religious environment. When you're told your whole life that being a woman and feeling horny is wrong and shameful you tend to grow up with some serious issues around the whole idea. The only thing likely to help her is going to a (non religious) therapist and working through her issues.. but that'll only happen if she wants to change.

Her insecurities are not normal and she is clearly in need of therapy, which- even if you were a therapist- would not be your job to fix. Healthy relationships don't tiptoe on eggshells like this, people don't use threats of terminating a relationship over small things like this. You know this but continue to see her. You can't control her actions, but you can decide who you date.
It's not your job to fix her. She was in a 20 year relationship with someone who destroyed her trust. That doesn't get resolved over night, and attempting to take that on is a battle that you couldn't win even if you were a licensed psychotherapist.

There is a big difference between lying and showing tolerance.
If someone was late, you wouldn’t necessarily make a big deal about it. He’s forgiving an exception in behaviour, not condoning its continuation.
The problem is now the pattern which no one would assume to happen. That he does need to correct and clarify. Where the line for forgiveness of being one time late and chronically late for a 1 month relationship, is a grey area.
